Velaris Integrated Interventions delivers a fully integrated, multidisciplinary model of care. Consultant Psychiatrists, Psychologists, Occupational Therapists and Case Management work seamlessly alongside our care and support

teams to provide coordinated, responsive intervention over an intensive 6 month period.

Velaris delivers rapid, evidence-based intervention initiated within weeks of referral, underpinned by an integrated neuropsychiatric, neuropsychological and functional formulation that informs a highly personalised rehabilitation strategy. Proactive risk assessments and risk management are embedded throughout, with close alignment to families, deputies and case managers, and a clearly planned step-down and transition phase to ensure continuity and sustained progress.

Focus: Production of an initial needs assessment report.

  

  • Exhaustive review of SCR, forensic, mental   health, and social history.
  • Operational risk screen and priority identification.
  • Communication with GPs, families, and Case Managers.
  • Formal INA report production

Focus:  Full MDT Assessment Domains and Care Plan Development. 


  • Risk formulation, Ongoing external agency contact, review measures.
  • Clinical Assessment, Mental State Review, Medication Management, Capacity & Legal, Crisis & Risk Reviews.
  • Mental Health/Behavioural, Cognitive Baseline, PBS Plan/Patient Passport, Therapeutic Suitability.
  • ADL, Personal Care, Cooking, Household Skills, and Road Safety Assessment.
  • Breakdown: Family system, community integration, and digital risk screen.

Focus: Targeted modular rehabilitation and clinical stabilization.


  • MDT Meetings: (4 sessions): Weeks 8, 12, 16, 20. 
  • Monthly Risk Governance: 4.5 months of SPM updates. 
  • Workforce Support: Staff Training & Reflective Practice. 
  • Coordination: Monthly Liaison and crisis management pool. 
  • Documentation: Mid-Programme Review Report. 
  • Risk Maintenance: Chronology updates & Safeguarding.

Focus:  Safe handover and long-term stability.


  • MDT Meetings (3 sessions): Weeks 22, 24, 26. 
  • Documentation (Final Discharge & Exit Report)
  • Monthly Risk Governance: Final profile review. 
  • Transition Liaison: Coordination of receiving services. 
  • Legal Review: MCA/DoLS finalization 

Care Team costs are charged separately and a quote will be provided as part of the INA and bespoke to the size of team, staffing ratio and sleep

hygiene requirements required for each client.


Four weeks care costs will be charged alongside the required Phase 2 cost and thereafter on a weekly basis from weeks 8 - 26.

formulation-led assessment of need that integrates neuropsychiatric, cognitive,

behavioural, functional and systemic factors, alongside a detailed and fully

implementable care and treatment plan that can be handed over seamlessly to

other teams without loss of direction or quality.


Clear clinical recommendations are provided for the next phase of rehabilitation,

setting out what should continue, what should stop and the level of support

required, supported by demonstrable progress against agreed goals with

transparent reporting of gains, limitations and ongoing risks.


This approach enables services specialising in longer-term or slower-paced

rehabilitation to take over with confidence, without the need to start again.
